Responsibilities of Patient/Caregiver We ask that you: Follow Hospice’s rules and regulations affecting patient care and conduct. Show respect and consideration of Hospice personnel and property, as well as other patients and their property. Provide a complete and accurate medical history, to the best of their knowledge. Help the organization understand their environment and provide feedback about service needs and expectations. Provide information about perceived risks and unexpected changes and complications that arise in the course of treatment. Follow the care, treatment and service plan developed and be honest with any concerns about their ability to comply. Discuss pain relief options with your nurse, asking what to expect regarding pain and pain management and working with your nurse to develop a pain management plan. Tell your nurse any concerns you have about taking pain medication. Help your nurse assess your pain, asking for pain relief when pain first begins, and tell the nurse if your pain is not relieved. Make it known whether you clearly comprehend a contemplated course of action and the things for which you are responsible. Ask questions if you do not understand. Accept consequences. Patients/families are responsible for the outcomes if they do not follow the care treatment and service plan. Provide Hospice with accurate and timely information to identify primary or secondary payers of the insured and promptly meet any financial obligation agreed to with the organization. SHOULD PLACEMENT IN A NURSING CARE FACILITY BE IMPLEMENTED, THE PATIENT’S BILL OF RIGHTS OR MUTUAL RESPONSIBILITIES OF THAT FACILITY WILL ALSO APPLY.
