Hospice of Holland Policies Patient Information Regarding Hospice of Holland Policies Advance Directives Hospice of Holland, Inc. recognizes that all persons have a fundamental right to make decisions relating to their own medical treatment, including the right to accept or refuse medical care. It is the policy of Hospice to encourage individuals and their families/caregivers to participate in decisions regarding care and treatment. Valid advance directives, such as living wills, Durable Power of Attorney for Health Care and DNR (do not resuscitate) orders will be followed to the extent permitted and required by law. In the absence of advance directives, Hospice will provide appropriate care according to the Plan of Care authorized by the attending physician and the Hospice Interdisciplinary Team and Hospice Medical Director. Hospice will conform to state laws regarding implementation of an advanced directive. Hospice will not condition the provision of care or otherwise discriminate against an individual based on whether or not the individual has executed an advance directive.Withholding And Withdrawal of Life Support Hospice of Holland, Inc. respects and protects the dignity and sacredness of human life and the right of the individual to make choices that are ethically and legally permissible. This policy addresses the withholding and withdrawal of all forms of medical treatment, including but not limited to, ventilators, medically provided nutrition and hydration, and antibiotics. Pain and symptom management needed to provide comfort to the patient will always be provided even if a decision has been made to withhold or withdraw other forms of treatment.
