Our policy is to support and protect the patient’s/caregiver’s fundamental human, civil, and legal rights; with impartial access to services regardless of the patient’s/family member’s race, color, age, sex, sexual orientation, creed, handicap, disability, height, weight, familial status, marital status, communicable disease, place of national origin, or ability to pay.

We will provide privacy; recognition of your dignity and individuality.

These rights and the Notice of Privacy Practice are provided to each patient/family and Hospice Employee, including volunteers. Additional copies are available by request and on the Hospice web site. Patient rights can be exercised by the patient’s representative on the patient’s behalf.
